This addendum is issued to modify or amend items in the Bid Documents for the procurement of the different projects of the Province of Cebu, as listed.  This shall form an integral part of the Bid Documents.

TERMS AND CONDITIONS:

b. Notarized certificate that the product is brand new not reconditioned/refurbished including parts and accessories.

c. Delivery and installation onsite should be done within sixty (60) calendar days

d. The system must be delivered ready to use.  Cables, connectors, adapters and accessories not specified but are essential for the unit to function must be provided free by the winning bidder.

e. Certification from at least two (2) government and/ or private hospital in the Philippines that they have been supplied the same brand being offered. (Letter Head must be from issuing Hospital). With Performance Certificate.

f. Undertaking to provide product orientation and operations training for end-user and troubleshooting training for biomedical engineers/ maintenance personnel.

g. Warranty and preventive maintenance should be two (2) years and one (1) years on parts.

h. Warranty periods shall commence from the date of acceptance by the end-user, testing and commissioning.

i. Provide regular quarterly preventive maintenance and calibration during the warranty period.
