ADDENDUM NO.  18 – 60

 

For:  Adv. No. 18-148  –  SUPPLY AND DELIVERY OF VARIOUS MEDICAL EQUIPMENT, FOR USE OF THE DIFFERENT HOSPITALS OF THE PROVINCE OF CEBU.

 

This addendum is issued to modify or amend items in the Bid Documents for the procurement of the different projects of the Province of Cebu, as listed.  This shall form an integral part of the Bid Documents.

 

  • TECHNICAL SPECIFICATIONS:

 

1.    CENTRIFUGE, CLINICAL… NO CHANGES.
2.    DENTAL AUTOCLAVE … NO CHANGES.
3.    EENT DIAGNOSTIC SET … NO CHANGES.

 -with Flex Mattress: 6” thick (standard size) & with leatherette cover

Standard Requirements

1. Manual in English Language

– Operational manual

-Original brochure with complete description of the equipment specifications

– Attach original manufacturers product catalogue and specification sheet. Photocopy/ computer print will not be accepted.

2. Valid certificate of ISO 13485 with consular authenticity

3. Certification from the manufacturer with consular authenticity that the brand has been in the market for at least ten (10) years;

4. Copy of Distributorship Agreement between the manufacturer and the local distributor (bidder) with consular authenticity;

5. Marketing Authorization, Registration Approval or Free Sale Certificate of the product issued by the Authorize Agency in the country of origin with consular authenticity.

6. Should be FDA/CE approved product.

7. GMP, ISO 9001

8. Shall meet IEC-60601-1-2:2001(Or Equivalent BIS) General Requirements of Safety for Electromagnetic Compatibility.

9. Certification that the bidder will provide a Service Unit that the end-user can use in case the equipment will be pulled-out for repair or maintenance within the warranty period

10. Certification to conduct preventive maintenance every quarter within the warranty period.

11. Should have local service facility. The service provider should have the necessary equipment recommended by the manufacturer to carry out preventive maintenance test as per guidelines provided in the service/maintenance manual.

12. List of important spare parts and accessories with their part number and costing.

 

Warranty and Service Support:

• Comprehensive warranty minimum of three (3) years and 5-year comprehensive annual maintenance contract after warranty. Warranty period must start on the date of acceptance by the end-user;

• Location, contact number of the Service Center of the supplier (bidder)

• Certification from the manufacturer to guarantee the availability of spare in the next five (5) years.

 

Delivery:

Terms and Conditions:

1. The supplier shall conduct a demonstration and testing of sample unit to the TWG Committee during post qualification evaluation (if required) within 5days upon the receipt of the letter request.

2. The supplier must coordinate with the Supply Officer of PGSO warehouse on the schedule of the delivery and installation at the recipient facility.

3. The supplier provides and installs necessary materials, accessories and miscellaneous components in order to ensure that equipment will be ready and functional.

4. The supplier informs the supply office once the equipment is installed in order to facilitate inspection of the Provincial Government Inspection Team.

5. The supplier shall provide comprehensive training staff and support services until familiarity with the equipment.

6. The supplier provides Invoice Receipt of Property, Certificate of Acceptance by the authorized personnel of the recipient facility and Users training attendance sheet to PGSO Supply Section as supporting documents for the payment. The supplier should also submit to the End-user/PHO the pictures/ photographs of the delivered equipments at the health facility together with the recipient in hardcopy and cds as an additional proof of delivery.

 
SINGLE LARGEST COMPLETED CONTRACT : 50% of the TOTAL ABC. SLCC: 50% OF THE TOTAL ABC OF BIDDED ITEMS.  ATTACH SUMMARY OF BIDDED ITEMS.

Initial Payment is Php 25,000.00, for refund base on total ABC of bidded items.
