This addendum is issued to modify or amend items in the Bid Documents for the procurement of the different projects of the Province of Cebu, as listed.  This shall form an integral part of the Bid Documents.

IV.  OBLIGATIONS AND RESPONSIBILITIES OF AGENCY

9. Shall provide the Shift-in-charge (SIC) his own means of transportation to effectively check and monitor the various guard posts/stations at all of the Province of Cebu’s facilities and  likewise shall shoulder the salaries and other benefits of said Shift-in-charge without additional cost to the Province of Cebu.  Shall also provide V.I.P. security to any of the Province of Cebu’s official on official business in Manila and any point of the country at no extra cost ;

9.     c) Benefits entitled to security guards/supervisor

      – submit certification under oath of additional benefits entitled for Security Guards and Supervisors to be deployed

2.       Posted security guard found drunk, drinking intoxicated liquor or smells of alcohol or found under the influence of prohibited drugs while on duty. Deduct from the billing of Php5,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php5,000.00.
3.       Providing confidential information to unauthorized person(s) Deduct from the billing of Php1,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php1,000.00.
4.       Security guard firing or fired his firearms indiscriminately (accidental / negligence). Deduct from the billing of Php10,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php10,000.00.
5.       Posted security guard observed playing with his service firearm or allowed others to play or tinker with his firearm. Deduct from the billing of Php5,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php5,000.00.
6.       Security guard apprehended for alarm, scandal or disorderly conduct within the premises of the Client, on or off duty. Deduct from the billing of Php5,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php5,000.00.
7.       Security guard engaging in mulcting or extortion activities. Deduct from the billing of Php5,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php5,000.00.
8.       Manifested or display of discourteous or rude manner or failure to render appropriate respect or courtesy to Client’s officials and employees or visitors. Deduct from the billing of Php1,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php1,000.00.
9.       Security guard found sleeping on duty. Deduct from the billing of Php5,000.00 and termination of the guard’s service upon receipt of the written notice from the CLIENT. Deduct from the billing of Php5,000.00.
10.    Guard performing duty for more than eight (8) hours without written permission from the Client. Deduct from the billing of Php5,000.00 per violation. Deduct from the billing of Php5,000.00 per violation.
11.    Failure of the security guard to report to duty without prior notice. Deduct from the billing of Php5,000.00 per incident. Deduct from the billing of Php5,000.00 per incident.
12.    Failure of the posted security guard to carry his license to exercise his profession as Private Security Guard. Deduct from the billing of Php1,000.00 per incident. Deduct from the billing of Php1,000.00 per incident.
